How do I apply for a Home Uplift as a renter if my lease is in my housemate’s name?

You can either help your housemate fill out the application or ask them to provide proof of consent.

Make sure attic insulation thickness is an R-30 rating (11” of fiberglass or rock wool or 8” of cellulose).
